"en": "<p data-HoCid=\"926933\" data-originallang=\"fr\">First of all, thank you for having invited us and for allowing me, as a community pharmacist who has worked in my field for 30 years, to come and share my thoughts with you. </p>\n<p data-HoCid=\"926934\" data-originallang=\"fr\">The Canadian Pharmacy Association strongly supports measures to increase patient safety, including increasing capacity for monitoring, surveillance and research; and the reporting of adverse drug reactions. For professionals, for pharmacists and for myself as a community pharmacist, patient safety is a priority. Safety needs to be part of the entire chain of events that begins when the prescriber orders a medication until the results are apparent in the patient.</p>\n<p data-HoCid=\"926935\" data-originallang=\"fr\">A strong system must be in place to ensure safe and effective use of medications\u2014one that includes a progressive early warning system for adverse drug reactions, post-market surveillance, and education of health care professionals. </p>\n<p data-HoCid=\"926936\" data-originallang=\"fr\">I have prepared a chart in order to explain the complexity of the process to you, which begins from the moment a problem is discovered, whether it be by a health care professional, a doctor, a pharmacist or a patient. You can see that the process is complex and has the potential for an adverse event and potential harm. You must always remember that the patient is at the centre of the process. </p>\n<p data-HoCid=\"926937\" data-originallang=\"fr\">We begin by recognizing the problem and beginning treatment, taking into account the analysis that has been done of the case and the patient's situation. Then, we implement was is called the health care plan and the treatment objectives. The information is given to the patient, who is supposed to use the drug according to instructions while watching for signs of improvement and adverse effects. The patient finds himself or herself managing the treatment and informing the professionals of what is happening. It is a cycle that repeats itself: the patient is satisfied or has adverse effects. </p>\n<p data-HoCid=\"926938\" data-originallang=\"fr\">Whether or not the medication is prescribed or non-prescribed, there are different actors involved in the process, including the patient. </p>\n<p data-HoCid=\"926939\" data-originallang=\"fr\">There are many adverse reactions. According to the statistics, from 37% to 68% of adverse drug events are said to be preventable. But in order to prevent these, the right decisions have to be made at each stage of the process that I have briefly described to you. Moreover, health care professionals are being asked to make rapid decisions, often with limited information or support. </p>\n<p data-HoCid=\"926940\" data-originallang=\"fr\">Patients have little support to be involved in the decision-making process, but increasingly, within the context we are discussing here, they are being expected to take on greater responsibility for their care, as well as that of family members. </p>\n<p data-HoCid=\"926941\" data-originallang=\"fr\">To help health care professionals and patients make better decisions, we believe education, information and tools must be readily accessible to them.</p>",

Un tel programme doit comporter un syst\u00e8me de d\u00e9tection h\u00e2tive des effets ind\u00e9sirables des m\u00e9dicaments, un syst\u00e8me de surveillance postcommercialisation et des programmes de formation des professionnels de la sant\u00e9.</p>\n<p data-HoCid=\"926936\" data-originallang=\"fr\">J'ai pr\u00e9par\u00e9 pour vous un graphique pour vous expliquer la complexit\u00e9 du processus, qui commence \u00e0 partir du moment o\u00f9 on d\u00e9couvre un probl\u00e8me, que ce soit par un professionnel de la sant\u00e9, un m\u00e9decin, un pharmacien ou un patient. Vous pouvez constater que ce processus est complexe et peut engendrer des erreurs ou l'apparition d'effets ind\u00e9sirables. Il ne faut jamais oublier que c'est le patient qui est au centre de ce processus.</p>\n<p data-HoCid=\"926937\" data-originallang=\"fr\">On commence par relever le probl\u00e8me et on instaure le traitement en tenant compte de l'analyse du dossier et de la situation du patient. Ensuite, on applique ce qu'on appelle un plan de soins et des objectifs de traitement. On transmet l'information au patient, qui doit effectuer un suivi, de toute \u00e9vidence, de l'efficacit\u00e9 du traitement ou de l'apparition d'effets ind\u00e9sirables. Ce patient se trouve ensuite \u00e0 g\u00e9rer le traitement et \u00e0 informer les professionnels de ce qui se passe. C'est un cycle qui recommence : le patient est satisfait ou a des effets ind\u00e9sirables.</p>\n<p data-HoCid=\"926938\" data-originallang=\"fr\">Que ce soit des m\u00e9dicaments en vente libre ou des m\u00e9dicaments prescrits qui sont en cause, plusieurs personnes participent \u00e0 ce processus, entre autres le patient.</p>\n<p data-HoCid=\"926939\" data-originallang=\"fr\">Il y a beaucoup d'effets ind\u00e9sirables. D'apr\u00e8s les statistiques, de 37 \u00e0 68 p. 100 des effets ind\u00e9sirables pourraient \u00eatre \u00e9vit\u00e9s. Mais pour ce faire, il faut prendre de bonnes d\u00e9cisions \u00e0 chaque \u00e9tape du processus que je viens de vous d\u00e9crire bri\u00e8vement. De plus, on demande aux professionnels de la sant\u00e9 de prendre des d\u00e9cisions rapides en se fondant sur de l'information parfois limit\u00e9e. </p>\n<p data-HoCid=\"926940\" data-originallang=\"fr\">Vous savez que les patients ne b\u00e9n\u00e9ficient pas d'un grand soutien pr\u00e9sentement pour tout ce qui touche le processus d\u00e9cisionnel, mais on leur demande d'assumer, dans le contexte qui nous r\u00e9unit ici, une part de plus en plus importante de leurs soins et de ceux des membres de leur famille.</p>\n<p data-HoCid=\"926941\" data-originallang=\"fr\">Pour aider les professionnels et les patients \u00e0 prendre de meilleures d\u00e9cisions, on pense qu'on doit avoir facilement acc\u00e8s \u00e0 toutes sortes d'outils : de la formation continue, des ressources d'information et toute la gamme d'outils d'intervention aupr\u00e8s du patient.</p>"
